MyChart Proxy | Summa Health (2024)

Proxy access enables, you (the Representative) to view and manage your family member’s (the Patient’s) medical records within MyChart. Depending on the access level granted, designated proxies can view information such as upcoming appointments, messages, test results and more. You can even view and pay your family member’s bills.Here’s how to set up proxy access based on your situation.

Granting Proxy Access to a Child Patient’s Account

  • For patients age 0-11, parents are granted full access to the child patient’s MyChart record.
  • Once a child turns 12 years old, parental access to the child's MyChart account will automatically be reduced to limited access due to state privacy laws.
    • Children can easily edit the parent's access level from within MyChart.
    • If the child consents, parental representatives can also request access from the child's care provider or the provider’s front desk staff member.
  • Once a child patient reaches age 18, all parental Representative access to the patient’s MyChart account terminates (due to privacy laws).
    • Adult patients can grant Representative parents access to their MyChart account by submitting a simple and easyproxy request through MyChart. You can also request access from the front desk or the child's care provider if the child consents.
    • If the patient consents, a Representative can also request proxy access from the patient’s care provider or the provider’s front desk staff member.

Granting Proxy Access to Another Adult Patient’s Account

To grant an adult Representative access to an adult patient’s medical record, the patient can complete a simple MyChart Proxy Access Authorization Form, which includes the patient’s consent to grant proxy access.

Requesting Proxy Access to an Adult Patient’s Account by an Adult Child or a Legal Guardian

The Representative can complete a simpleMyChart Proxy Access Authorization Formand provide proper identification and supporting legal documentation as required, such as a court order or durable power of attorney for health care (DPOA).

How to Set Up Proxy Access

Step 1: Both the Representative and Patient must have a Summa Health MyChart Account.

If the Representative has received care from a Summa Health care provider, there are two ways to sign up:

1. Sign up online here.

or

2. Have your care provider or the provider’s front desk staff member send you an activation link at your next visit.

If the Representative has never received care from a Summa Health care provider:

Go to MyChart self-sign-up and submit a quick 3rd party identity verification in order to create a patient record.

What if the Patient is not able to submit a proxy request?

A Representative can obtain proxy access from the Patient’s care provider or a provider’s front desk staff member.

You should receive an email notification that your request has been completed within 7 business days after we receive your request.
If you do not receive an email notification within that timeframe, please contact Health Information Management at 330.375.4941 Monday - Friday 8:00 am - 5:00 pm for assistance.

What is proxy access?

Proxy access and allows a parent (or guardian) to log into their personal MyChart account and then securely connect to information regarding their family member.

Proxy Access to a Child's Account

  • If your child is age 0-11 you will then be granted full access to your child's MyChart record.
  • Once your child turns 12 years old, access to your child's MyChart account will be automatically reduced to limited access (due to privacy laws). In the case of an adult Representative accessing a consenting adult's record, such as a spouse, we ask that the Patient provide the MyChart Proxy Access Authorization Form (the form includes your consent for this type of access).

In the case of an Adult Child or a Legal Guardian Requesting Proxy Access for an Adult

We ask that the Representative bring in the MyChart Proxy Access Authorization Form with proper identification and supporting legal documentation if required, such as a court order or durable power of attorney for health care (DPOA).

May I request proxy access for more than one person?

Yes, you would follow the same process for each qualifying individual.

Can I ask questions regarding a family member from my MyChart account?

No. MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can my spouse and I share one MyChart account?

No. Due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

Is a Proxy Representative responsible for a Patient's bills?

Proxy access does not make you responsible for the bill. The guarantor is the person responsible for paying the bill. You can find the guarantor name and account number on your statement.However, proxy access does provide visibility into the patient’s bill. You can view and pay their bill online.
